Our routine practice employed the maximal 80 μg ACh into the RCA and 200 μg ACh into the LCA not to misdiagnose the patients with coronary spasm. At least from five to seven times procedures are necessary during ACh spasm provocation tests. Radiation exposure and the adverse effect of contrast medium is one of the problems.

Clinical usefulness, angiographic characteristics, and safety evaluation of intracoronary acetylcholine provocation testing among 921 consecutive white patients with unobstructed coronary arteries.
BACKGROUND Coronary spasm can cause myocardial ischemia and angina in patients with and those without obstructive coronary artery disease. However, provocation tests using intracoronary acetylcholine administration are rarely performed in clinical routine in the United States and Europe. Thus, we assessed the clinical usefulness, angiographic characteristics, and safety of intracoronary acetylc...

As pharmacological spasm provocation tests, acetylcholine (ACh) [1-3] and ergonovine (ER) [4,5] are employed in the cardiac catheterization laboratory. However, we often encounter the major and minor complications during performing these procedures. As a diagnostic tool, we should perform spasm provocation tests more safely without major complications. Multiple and proximal spasm documented by ...

متن کاملImpact of old age on clinical and angiographic characteristics of coronary artery spasm as assessed by acetylcholine provocation test
BACKGROUND Smoking and other risk factors have been well known as important factors of variant angina or coronary artery spasm (CAS). However, clinical features related to age on coronary artery spasm have been rarely evaluated. METHODS We evaluated 3155 consecutive patients with insignificant coronary artery lesion. Patients underwent Acetylcholine (Ach) provocation test for induction of CAS...
متن کاملClinical implications of provocation tests for coronary artery spasm: safety, arrhythmic complications, and prognostic impact: multicentre registry study of the Japanese Coronary Spasm Association.
AIMS Provocation tests of coronary artery spasm are useful for the diagnosis of vasospastic angina (VSA). However, these tests are thought to have a potential risk of arrhythmic complications, including ventricular tachycardia (VT), ventricular fibrillation (VF), and brady-arrhythmias. We aimed to elucidate the safety and the clinical implications of the spasm provocation tests in the nationwid...
